I have a fair knowledge of vb, but am relatively new to ASP although it seems to have some similarities. I have setup a MySQl 5 db and a DSN. From vb projects inserting and select statements work fine. So the connection works.

Now I really would like an example of how to create a form in ASP, let's say with 3 textboxes: FirstName, lastName and Phonenumber

How would I create HTML with ASP and a submit button so when I click submit it writes whatever is in the textboxes on the HTML form directly into the MYSQL? I would appreciate suggestions on both the present DSN connection as well as a DSN-less example with connectionstring

If it isn't too much trouble please add some radiobuttons or checkboxes to the example so I can see how to work with that as well. Say for example that a checked checkbox writes to a column named KeepMeInformedOfUpdates the value of 0 or 1 of TRUE or FALSE or whatever.
